About the Role:

  1. Take vital signs (temperature, blood pressure, pulse, and respiratory rate).
  2. Provide direct patient care such as assisting with bathing, eating, dressing, and walking patients.
  3. Turn and reposition bedridden patients to prevent the breakdown of their skin.
  4. Change bed linens.
  5. A record amount of oral intake and measure urinary output.
  6. Transfer patient using Hoyer lift or assist patients in transferring from bed to chair, chair to chair, or chair to bed as necessary.
  7. Assist patient in ambulation with/or without assistive devices such as walkers and canes
  8. Position and empty bedpans and/or urinals as necessary.
  9. Interact with patients and family.
  10. Transport patients (if allowable) and equipment as needed per Agency policy.
  11. Works with personnel of other community agencies involved in the patient’s care as directed by the staff nurse
  12. Performs routine housekeeping tasks as related to a safe and comfortable environment for the patient, as instructed by the professional nurse
  13. Attends in-service training and recertification training as required by state and federal regulations.
  14. Confirms on a weekly basis, the scheduling of visits with the Homecare Coordinator, to coordinate necessary visits with other personnel.
  15. Notifying the Agency of absences due to illness, emergency leave, normal vacation periods, or special professional meetings, which will affect agreed to service with the Agency. (Notification must occur 4 hours or more prior to visiting scheduled.)
  16. Demonstrates patience, honesty, good health, and the willingness and ability to follow directions.

Requirements For The Role:

  • High School Diploma or GED.
  • Completion of a qualified, state-approved training program consistent with the state and federal requirements and for which an HHA, CNA, GNA certificate was obtained.
  • Satisfactory performance on a competency evaluation and annual reevaluation.
  • Has the emotional and mental maturity necessary for establishing and maintaining a good work relationship with the patient, patient’s family, and the personnel of the Agency.
  • Has a reliable means of transportation to get to the assigned client.
